Every resident has a care package put in place which includes a person centred plan of care, risk assessments, moving and handling assessments and mental health assessments. Residents will be assigned a qualified, experienced keyworker within one month of admission. We encourage relatives and Social Workers to have active input into each aspect of the unique plan of care. All Care Plans are reviewed by the Keyworker, resident and Management.
Our linked GP is Nethergreen Surgery, Dr Humphrey Emery is our current GP and visits Bridgedale House once a week to hold an in house surgery and discuss any medical needs with residents and relatives. Dr Emery will also visit during the week for any acute medical concerns between these times. All of our residents have formal medical care plans put in place at the time of admission to ensure that we meet with their specific and individual medical needs.
The District Nursing Team will assist with any nursing requirements as required.
The Chiropodist visits Bridgedale House every 6 weeks.
Our linked Dental practise is Katherine Lines at School Road, who visits at Bridgedale House or the staff will support with visiting the surgery for treatment.
All residents that attend hospital appointments will be supported by our care staff and transported privately as required.
